How to Order
Only order through this form if you are paying with purchase order or check, or if you are tax exempt. If you will be paying with credit card, please utilize our “Shop” page. Please click the button below and complete the fillable Product Order Form. Once you have completed this order form, please save to your computer as a PDF and attach it to the “File Upload” field below. Please complete remaining fields, and hit “Submit”. Upon receipt of your order form, Kristin will email you your invoice. Taxes and shipping costs will also be added to your invoice. If you are tax exempt please email Kristin proof of exemption at kristinsouers@gmail.com. Your order will be shipped upon receipt of payment.
